The fact that, unlike the US, Germany and many other countries with low birthrates, they don't allow immigration which will hurt them further in the long run. That being said, I'm not in Japan.

As the population declines there are fewer workers and more (elderly) dependants. The fact that Japan has one of of the highest life spans exacerbates this problem. Immigration helps to offset this by bringing in a new workforce. Even if they suddenly had a baby boom it'd be roughly 20+ years before they'd be able to enter the work force. They currently sit at ≈65% of their population as dependents. By 2035 that'll be closer to ≈70%. The US, by contrast, is currently at ≈40% dependants and that's expected to drop to around 30% in the same time frame, largely due to immigration (assuming it doesn't royally screw itself with bad immigration policies). Automation will help keep Japan's productivity up but the burden on individual families, social security and pensions will be higher, which will likely reinforce the low birthrate. The idea behind not allowing immigration is to preserve their culture. The irony is, they're actually killing it. It's like inbreeding to preserve a bloodline. Might seem like a good idea now, but down the line it's not gonna be pretty. On a side note, they are slowly adapting to allow more foreign workers but refuse to allow full on immigration. Over time we will likely be seeing them rethinking their current policies.

Only 23 million, it's just that it's an island where most people don't live in the central mountainous region. Essentially its a population slightly lower than Texas in an area slightly larger than Maryland where most people live in one third of the space. The end result is that my city (the second largest in the country) has a population density 80% higher than New York.
