1. That's basically it. Explore to the best of your abilities, take note of your surroundings for shortcuts, hidden paths, etc. In dark souls 1 it was entirely possible to go to locations out of order, and often in that aspect was much more open world, whereas the later games usually have a more linear progression (though you still have to find the branching paths and secret areas on your own).
For instance, in Dark souls 1 you supposed to ring the two bells. The crestfallen knight tells you where they are, but does not direct you to them, only saying one is up and the other is far down. By moving in one direction, you will eventually have visual of only one path that goes up, and beyond that you can see what appears to be a cathedral. By being observant, you can properly find your way.
2. Multiplayer is still the same. Put down a sign, get summoned to complete the one area, and then move on. They have made it considerably easier to do this in the 3rd game, by setting a password that matches you only with others with the same password.
PvP is done in 2 ways. The first is invasion, forcing your way into other players' worlds to PvP. This is involuntary, and interrupts the host, though in Dark souls 3 it is heavily weighted towards only invading those who are already co-oping. The 2nd way is by placing down a red soapstone, which allows players to willingly summon you for PvP. This usually involves an honor code, and unless trying to have a fight club should always be 1v1.
There is also a complex rivalry system this time around, as well as the mad covenant, the mound makers, but those are just variables.

What would proper form be for a willing duel?

Typically, 1v1, No estus use, No running and hiding behind AI enemies, No cheating (hacking, disconnecting when about to lose, lag switch, etc) Optional but usually done: a proper greeting before fighting (bowing, waving, "hello" carving, etc) do not hit someone when they are greeting you. Other than that anything is ok. You can use whatever other consumables, weapons, armor, etc You wish. Healing spells are an iffy subject, though many would probably allow them as they are incredibly easy to stop as opposed to estus which is near instant. I would probably avoid them though anyways. In one player breaks the honor of the fight, anything and everything is fair game.

Healing spells are usually a go for me, however I typically run around with a blessed shield, the sun princess ring and a blessed longsword. Since my main is a faith build, the blessed enchantment gives me an A rank on that sword, and all the regen makes me a bit harder to kill. Though usually I only pull out the shield and the sword if the honor code is broken. Otherwise it's a talisman and a lightning dark sword. I have no real pvp build since I don't willingly pvp xD

I was wondering but I know you don't attack someone while they use their emote but what about when they cast a buff spell, not an attacking spell. Do you let them get through the animation or attack.

Good question. I would think that at that point it's ok to attack them. If they rebuff in the middle of a fight them it is certainly ok to attack them. At the beginning though, I am unsure.

Probably not that common, but me and my bro were dueling and he uses deep protection, so I'd always let him use it. The results the same anyways haha.

Well all of that stuff seems pretty common sense to me. As the participant of... a fair share of proper shotgun battles in Gears of War, I'd be happy to abide by these rules, lol :P
