What's with me getting constant "False Upgrade Notifications"? I'll be playing and I'll be killing enemies and a notification pops up on the screen, with a picture of a piece of equipment, with a little green circle with a plus sign inside and a banner above it that says "Upgrade Unlocked" . I'll go to my menu, check my gear and there isn't any actual upgrade that's been achieved. I keep getting this a lot! It happens when killing enemies and when turning in bounties.
