[spoiler]my vote would have to go with Bloodborne. I love the gothic Victorian-style town, and gives the game a creepy, and unnerving look, which perfectly complements the difficulty of the game[/spoiler]

The funky blend of 50s, pseudo sci fi from the technology pre war, and post apocalyptic are great, and then adding in all the DC area landmarks on top of that. I think, truthfully, it would be a tie between that and Rapture, that was a stunningly realized world. Edited for phone typing.

Final Fantasy X. Visually it's really bouncy and cheery for most of the game but once you start to understand the context of the dialogue and characterization it becomes a very emotional experience and the work around you starts to reflect it.
