Name would probably be for if they sent you something in the mail, gender is probably just for research purposes. No idea. Email is probably just for contact purposes. I believe Bungie have said that it is for a variety of projects. The V1 survey was sent out via email if I'm not mistaken. Yes, they might call people into the studio that live within a reasonable range. For sending beta codes maybe?

So Bungie knows how much of their audience is female, and they can compare female results from the survey to male ones. Name, who knows, maybe just to know you're legit? Out of the thousands of gamers who are doing the survey, not many get to actual participate in anything Destiny related (at least not yet). The survey is good for basic user research. I can say with 100% certainty that the gender thing is so that they can gain an accurate representation of how many girls are on the site, and then plan accordingly how to remove them.
