Halo 3/ODST/Reach, Minecraft, Skyrim and Fallout. But most games these days are pretty easy even on hard, as hard now roughly translates into "slightly more difficult than normal" in the minds of developers.

Honestly, most new games released these days are pretty easy even on the highest difficulty. This mainly has to do with developers just thinking that making enemies have more health, do more damage and shoot more often equals a harder game. I would say the easiest I can think of at the moment would be Skyrim. The game is only slightly challenging at the beginning, but once you get to around level 40 you're pretty much a demi god. Not to mention how broken enchanting and smithing are. You can pretty much make armor and weapons that will be better than ANY you will find spawn in the world.

Some older games are absolutely brutal. Most Halo games (even Reach) actually adjust the AI's intelligence in addition to making shots from enemies do more damage while making your own do less.
