How do you get the CQB helmet?
Me and three friends all made it our goal to unlock the CQB helmet, which they said is unlocked by beating the campaign on Heroic... But when we did it, they all got the helmet and I didn't. What else do I need to do?

you get it via Halo Waypoint. You can find the description of how to get the helmet there too.

You need to DL and sign into Halo Waypoint on your 360.
