I've been doing normal Vault of Glass on all 3 characters when possible, which isn't easy since hardly anyone needs/wants to do it, trying to get the Glass Minuet and it still hasn't dropped! I've gotten the bloody Crux of Crota to drop more than the Glass Minuet! RIDICULOUS!
