Since House of Wolves came out and patches after that I have fully upgraded a Thorn, a Dragon's Breath and a Mida Multi-Tool (this one ascended) with two different characters and so far no achievement popped. Made the unglitch method and its not working. Wonder is HoW or latest patch definitely glitched the achievement or is there is a different method to make it pop.
