Moon of Osiris Emblem
At the end of a trial game I got the Moon of Osiris Emblem and watched it appear on the scoreboard and everything. I kept a slot open for any emblems that I got but even after loading to orbit for the next match I did not have the emblem. Not really sure if you can do anything to validate my story, but it happened.
