Early quitters glitching commendations
When a player leaves the crucible match, in this case iron banner, before the commendations screen appears it locks all the other players out from giving and receiving commendations. The character belonging to the person who left is not displayed but their name and icon is, which causes all others to not line up with their name and character model.

Guess what? Nobody cares! Who cares about accommodations??!!! It's a worthless system anyway.
