This is a bug caused by lag or bad connection. The death seems to occur because the dead person's console communicated their position to the host too late, therefore the game still believed them to be outside the room and killed them.
* The first thing you should do if this happens to your team is check the bars for red and yellow in the roster.
* You can solve this issue by going to orbit and returning (check to see if the connections have improved at this point) - the game selects the best host when you enter the activity.
* You can also somewhat prevent this issue by having everyone in the fireteam when you enter the raid from orbit. If you have someone new join and you have bad connections, going to orbit and coming back in can often solve them, if its a problem.
If going to orbit does not fix the problem, it may be that someone in your fireteam has a very bad internet connection that can cause a delay in their position being communicated to the host which makes them die. In this case, you have a few solutions :
* That person is a self rez warlock, they can die voluntarily by falling in a pit and self rezzing later (you can not self rez if you are killed due to being ALIVE outside a safe room), but you can do it if you die due to another reason.
* That person should get into the room VERY early, to give their connection more time to communicate position to the host.
* Ask your teamate where they are located and what kind of internet connection they have. If its a wireless connection, see if they can use an ethernet cable. Wired internet is always faster and more reliable.
Hopefully this will help out some of you. Obviously this is a much bigger problem on hard mode than normal mode where you can just rez the dead guy.
