Heavy Ammo Packs Consumed During Cut Scene in Dark Below...
... fail to register. See video for details. Ammo pack consumed during cut scene but rocket launcher empty when the game kicks in. Ammo cool down is in place too so I can't just grab another one.

It's a known issue , has been since launch and the still haven't fixed it.
