So, let's talk about this. On Vanguard and especially Heroic strikes people regularly drop out. It didn't used to be a problem, and while it annoys me greatly I get it. But ever since TTK dropped, I have to wait forever for anyone to be matchmade back into my group, if it happens at all. When I come into a strike in progress it is always on the last 25% health of the final boss. That seems to be the only time people are regularly matchmade into strikes in progress. This is incredibly stupid and kinda defeats the entire purpose of matchmaking, not to mention destroying the idea of vanguard streaks.
And the Crucible. I'm not big on PvP, but when I do play, the matchmaking encourages me to return to PvE quite quickly. Nearly every time I get matchmade into a game with a team that wins, following that game the system decides that the teams were too uneven and breaks them up to make a more even match. Then the next 3-6 matches are all with me on a losing team, usually a drastically losing team. Then the process repeats. This is beyond ridiculous. I am so sick of this crap. It is nice to have a decent game, maybe once every ten or twelve matches. I would love for it to happen more often.
TL;DR: Strikes playlists almost never add people to ongoing strikes and Crucible matchmaking is usually severely out of balance.
