Cardinal skin. You get it from completing the 3rd level of the explosions challenge (kills with MIRV grenades). If you look on the borderlands wiki for character skins, it should tell you how to obtain each one.

I'm pretty sure it appears at the vending machine, as well as the slot machines at moxxis.
