How I solved my centipede and bee/fly/lion disconnects
I had been getting the above 4 errors every 10 minutes since late last week. I was unable to do any strikes or PVP it sucked, i was 1 strange coin away from buying the damn SUROS!
I tried resetting all of my equipment (console, router and modem) port forwarding, I tried DMZing I even bypassed my router completely and plugged my PS4 into my cable modem. Finally last night (after the 9/30 stability server side patch) I called my ISP (Time Warner in Rochester NY) and explained that I was having connectivity issues due to data loss. The service rep found ~%4 packet loss between him and my modem. We tried switching Ethernet cords (ps4 directly connected to the modem) and the data loss stayed constant. He scheduled a time for a tech to come look at the cable hookup in my basement.
Then the magic happened.
I told him I am going to try plugging my router back in. He said he will remotely restart my cable modem. BOOM no more packet loss and no more disconnects.
TL;DR after 5 days of constant centipede and bee/fly/lion disconnects the problem was solved by having my ISPs techsupport remotely restart my cable modem.

I have done that before and was not a fix for me. My ISP found that there is no packet loss with me connection so the reset did not do anything for me. Still run into plenty of errors and see a few that a I haven't before.
