The most common cause for this issue is having a full consumables inventory. A work around, for now, is to deposit a consumable into your vault.
Please note that using ammo synthesis will not be credited during cutscenes or while downed.

No....this is incorrect info. I have no idea why people spread this BS as this is innacurate The CAUSE of the issue is having MORE THAN ONE of the same ammo synth in character inventory. Make sure you don't have more than onestack of heavy synth on your character; put the rest in the vault and refill when needed.....
