Not a glitch. BWI can be awarded directly when killing a blade of Crota. Also, it took me 83 blade of Crota kills to get Husk.

Does it drop as a tetrahedron or the pentagonal thing?

It isn't a drop. It just goes directly into your inventory like awards do after raid bosses. It will show on the side of your screen but isn't something you need look on the ground for. After a kill, you automatically get it or you don't and try again.
