The Dead Ghost on Mars (Off World Transit) and the Dead Ghost on Earth (Bunker RAS-2) seem to be fixed for me.
The Dead Ghost in the Lunar Complex on Earth still is glitched. Checked my Grimoire and I'm still missing it's associated Ghost Fragment: Darkness 3.
My Dead Ghosts checklist in the Grimoire is at 61/62.

My Darkness 3 is also still glitched. Both the RAS-2 and Mars ghosts are no longer bugged.

The others all work fine now, but I am sure I picked up the one in the lunar complex and yet I'm missing that entry. It's such an important one too... :-/
