In the shrine of oryx mission, the part just after the long winding tunnel with the pulsing blue light. You come round a corner and there's a few dregs fighting some acolytes, keep going until an ogre pops out, kill it then die, the checkpoint should be just as you entered the area :)

Nothing I'm aware of, there's always the cave next the helicopters in skywatch, but the knight in there seems to spawn more frequently than the ogre :)
