I understand in PvE that the enemies get knocked back and have an animation and so on, but this is clearly not the case in PvP. Does impact from your weapon make their weapon more difficult to aim if they are being hit? That was my guess.

I am not sure. I used "Doctor Nope" in a PvE dungeon. Hits without the increased impact perk on the rifle did the same as hits with the increased impact perk. So either the perk does not actaully work (wouldn't suprise me) or impact has no effect aside from a basis of comparison of weapon traits.
