This was completely his fault and unavoidable:

You can lock store purchases behind a password and you shouldn't have your credit card info saved if you let children have access to your account.

If the auto renew was off it shouldn't have charged him and if his son downloaded something after it should have been turned off then he shouldn't be charges

Auto renew was not on, but the credit card info was still on the account. So the child goes to the store and sees the PS+ games. They try to download one, but it says you need PS+ and takes them to the purchase screen. One press of the X button, and they have subscribed and downloaded the PS+ game. The password protection would've prevented this, or just removing the credit card info to begin with.
