Is it as difficult to have the police chase you as Totalbiscuit says?
(Also disregard missions where the police are supposed to chase you.)

Depends on your "reputation". Since mine is full to the positive end I can get away with so much shit. But when I do get chased it's easiest to just get on a train. The Police just hate trains for some reason

Kill a few people, wait till the ctOS tracks you, then your good to go.

Does the game wait for civilians to call the police or CTOS actively tries to track crime you commit. TB did run a few people over and break somethings in the vid, but no police were called on him until he carjacked someone. Seems odd.

Eh, I accidently met them a few times when I stay stationary for too long. Not that fun as they are pretty good with getting in your way. The gangs on the other hand are worse. As you usually have something else to do while they chase you
