To get the achievements I played on heroic with Tough Luck, Fog, Famine, Thunderstorm, and Catch. Never had a problem breaking 50-60 k and ran through the points achievements first try. Great combo of 5 skulls without making the game play so hard that its annoying. [Edited on 10.03.2007 11:03 PM PDT]
