Grasp of Avarice Ogre trigger bug?
I have run this dungeon 13 times so far and have only ever seen this entrance once - is this a bugged encounter trigger, a rare entrance, or something else entirely?

I've only seen this once, too. Maybe it depends on how fast you get through the previous room?
