Wow, no wonder I've been spending a fortune in glimmer on heavy synths. I lose a rocket from my Dragon's Breath upon every respawn (death, join fireteam in different area); I haven't tested cut-scenes yet, but I suspect it will likely occur there as well. Will edit once I find out. It only seems to happen on that particular weapon. Not on my MGs, nor my Unfriendly Giant. Apparently quite a few people have been experiencing this on other weapons/gear. Fix this, PLEASE!!! Those synths are expensive!
