Do you think if a Vandal botched up an important mission or failed to complete an objective, would it have its arms removed again, returning it to dreg status, or would it be killed outright by the higher ups?
Any thoughts?

I'd say since removing arms is a huge sign of shame, they'd be more likely to remove the arms and force dreg status. Unless the Fallen are numerous in a specific House/Clan, I don't think it's wise to kill off their own. Just makes their House weaker. The shaming of no arms seems to be more practical and possibly a motivation for them to earn back their "arm status".
