FINALLY, someone with the same controller-issues...my heavy MG sometime stops firing but I'm sure I didn't let go of the trigger (and no: it didn't ran out of ammo)
Also have the lag on kill confirmations and deaths behind cover...

Is it possible you are accidentally clicking the left stick and initiating sprint? I have this problem frequently while trying to evade gunfire while still shooting as it makes you stop firing (even if you don't actually push forward to run). I actually just made a post about this, an easy fix would be to make sprint NOT override shooting. But it will be up to Bungie to fix this with an update.
