Depending on how surreal we're allowing this to get, I did have a DnD NPC at one point who could torture people by putting a small sliver of a crystal in someone's veins, then command it to slowly grow throughout their entire circulatory system until they talked...

His name was Tacitus. He was a man who through magical BS got merged with a golem made of crystal. He lost all of his humanity that day, and has almost no recollection of his life before the event. He is a near god amongst men due to his sheer strength, fortitude, and general capability... Does not give enough of a crap to leave his home 90% of the time. The only times he tends to leave are because some unfortunate creature got too close, or because another recurring character (Alexander) literally picked him up and carried him off toward adventure. I literally have to roll a "Tacitus give-a-sh**-o-meter" D20 just to see if he does indeed give sh**s.
