I'm confused.
How is the Mako back, but it takes place in another galaxy? Wouldn't another galaxy mean completely different societies, aliens, and technology?

The only logical explanation after the disaster that was ME3 (we need a documentary on what the -blam!- these guys were thinking) is to retcon everything. Anyways anyone getting excited at this is a chump that's wanting to be burned. It's an EA published game with the Bioware logo. They might as well put a biohazard warning sticker on the game.
