This is a pretty solid list. Well done.
With Hardcore mode, I'm very interested to see how they do it. This is very similar to Halo's Skulls, but with this being a shared world experience I'm not exactly sure how they would implement it. Granted, most of the ideas you put forth could be individually instanced if you will so it certainly is possible. As well, you could make Strikes/Raids work in the same fashion as Halo's Firefight skulls and whatnot.
Difficulty in general is a delicate concept to deal with as Bungie has stated multiple times they want us to be able to play with our friends and not let character level disparity create unnecessary borders.
For your defense mission, I certainly hope they have a day (or multiple days) where events happen surrounding the Wall. And they could be phased events. Imagine that the Fallen have gained a foothold near the last city on Earth and they start pounding on our defenses. Phase One could be epic if it were a 3-5 day event with small instanced public skirmishes interspersed around the area. You could take out one in a ten-min battle, only to have another one crop up two minutes later. The meta-challenge which could be linked to Bungie.net is that once 100k skirmishes are destroyed we then go on the offensive and take it to the Fallen Forward Operating Base for Phase Two. We then battle it out there for 2-3 days until they're knocked out. Whoever participates in a phase gets a reward and if you participate in both phases you get three rewards.
