Something along the lines of this is how I picture him.

Also, Moddey Dhoo. Not quite Death, but he is in the same occupation.

I remember reading a short story in a Christmas book that had Death in it. He appeared as a hitchhiker in a blizzard and said some interesting stuff about how he imagined hell to be a cold place. Although he was sinister, he wasn't overtly "death-y" or evil or anything, and I liked that.

Edited by Sandtrap: 1/1/2014 10:36:30 AMI liked this interpretation of him by some random artist. I think maybe it's because the artist did a backwards take on him. Rather than terrifying you in your final moments, he shows up as a friendly entity. He'd probably walk with you for a bit, explaining things to you in the best manner he could. Hell, even looks like he'd buy you a round of icecream.

Death, the Pale Horseman, from Supernatural.

This by some random artist. He has white wings showing that he's an angel, garbed in black, intimidating, but also human looking.

As in the Grim Reaper, Death the Horseman, or the Angel of Death?
Yeah, I'm fairly sure Grimmy and the Angel are the same, but they've been changed with pop culture so can be counted as separate entities.
