I've had similar instances in the past, but I don't seem to be having the exact issue. It used to be that after so long all abilities would stop working. Supers wouldn't trigger, grenade animations would play, but no grenades would appear. Melee wouldn't trigger the ability and so you just couldn't melee at all. Weird times.
