The achievement states: "Complete a Raid without anyone in your fireteam dying".
I was curious about this most tricky achievement/trophy, so I did a little experiment. I understand people can solo kill Atheon with two grenades, however, this will be rendered impossible when the hotfix goes live. So what I did was find a small group with the Atheon checkpoint, then have *cough* internet problems *cough*, and try to solo Atheon with my Two Grenade Sunbreaker Sunslinger Warlock (long name I know). After about an hour of figuring it out for myself (with guidance from a couple videos I watched a week or two ago), I was triumphant. Atheon was walking on molten fire all the way off the cliff. Now, Mission Complete comes up, seconds clock still stuck at zero (not at all annoying it isn't fixed yet), but no achievement. I go to orbit, open menu, log out/switch profile, log back in, still nothing. Quit out of Destiny, turn system off, turn system back on, log into Destiny, still nothing. The Post-Game Carnage Report is where things get.....odd. I have linked the report below. As you can clearly see, my fireteam had one kill and zero deaths, thus completing the stated requirement from the top. Why do you think my experiment didn't work?

You gotta complete the whole raid(Vex plates to killing atheon) in one session without anyone dieing, going to orbit(includes disconnecting) or joining after game started

you need to finish the FULL raid without n e 1 dying. using a checkpoint wont work, gotta go from beginning to end .

Sounds like you got what you deserved for manipulating a glitch. But to answer your question, deaths are recorded from checkpoint to checkpoint. Therefore from the beginning of the raid its tracking the deaths. When somebody dies during anytime in the raid, that achievement is locked for that entire save.

Give it a few days I've noticed my achievements don't pop up till a few days gone by shit I waited almost a week for "strength in numbers" or whatever it's called

Either you really need to do the entire mission with a fireteam, or just using a checkpoint over and over doesn't register as an entire raid. Quite simple reasons but hard to explain.

You have to do the raid from start to finish plus your team mates leaving or getting disconnected kind of counts as deaths it just doesn't show. If you want the trophy then you have to put more effort in it than that

If you look at the carnage report there are yellow boxes indicating stages in the vault. All need to be completed without dying. If they need to be completed consecutively I do not know.

The Templar oracles, the Templar, the Gatekeeper(s) phase, Atheon. I figured this was obvious. Of course just killing Atheon doesn't mean beating the raid.

The timer at 0 isnt a glitch, you just killed the keeper of time and space so you should be glad that it didnt do more than stop a timer at 0

When i have an achievement that won't unlock that i know i should have, this is what i do. You have to have Destiny running and be in orbit while you do this.

If you die ONCE at any point from start to finish, you have to reset and start again.

Did you do the raid in one sitting? If you quit half way through then go in again it counts as a death.

My guess is that if anyone in the fireteam you join had died it won't pop since technically someone HAD died. Basically you'd have to complete the raid without anyone dying at any point.
